Q. A 75-year-old ship male presented with shortness of breath, cough, chest pain and fatigue. Additionally, he has also lost 10 pounds over the last 6 months. The patient states that the symptoms are getting progressively worse. He is now retired after owning and operating a roofing company for 30 years. The patient recently returned from a trip to India where he attended his brother’s wedding. Blood pressure is 130/88 mmHg, heart rate 85/min, and respirations are 22/min.
His physical examination is unremarkable with the exception of clubbing of the fingers. On chest x-ray, multiple nodular pleural lesions are noted.
