Q. A 25-year-old victim from a motor vehicle accident was brought in by an ambulance. He was given oxygen and IV fluids on the way to the hospital. He is conscious and alert. On examination, blood pressure 110/70 mmHg, heart rate 120/min, respiratory rate 34/min, and SaO2 88% on oxygen via nonrebreather mask. Chest and abdomen examination reflected bruising. Neck veins are not distended, trachea is midline, and decreased bibasilar breathing sounds are heard on auscultation of the lungs. Right chest moves inwards upon inspiration and outward on expiration. Abdomen is soft and non-tender with normal bowel sounds. What would be the most likely diagnosis?
