Q. A 30 year-old woman with a longstanding history of epilepsy with generalized tonic-clonic seizures comes to see you for consultation prior to becoming pregnant. She is currently taking levetiracetam for her seizures and wants to know if it is safe to continue taking. She was previously taking lamotrigine but switched to levetiracetam six years ago when she developed a macular rash with a dose increase in lamotrigine. Since being on levetiracetam, she had one seizure more than five years ago when she was taking 500 mg BID but has not had any further seizures since her dose was increased to 1000 mg BID. Vital signs are normal. Neurological examination is unremarkable. Cardiopulmonary auscultation is normal. In addition to adding folic acid 4mg per day to her regimen.
