On average, 1 hour after the application of 5% topical cocaine, the abnormal pupil in Horner’s syndrome will dilate less than 0.5 mm whereas the pupil in physiologic anisocoria will dilate 2.1 mm. (Note that cocaine from this testing can be detected in urine samples for at least several days.)
