Epley Maneuver

  • At the completion of the 2nd step of the Dix-Hallpike maneuver, the patient is hanging his/her head backward off the exam table with it laterally rotated 45 degrees.
  • In the 3rd step of the Epley maneuver, the patient should laterally rotate his/her head in the opposite direction 90 degrees, so it is now laterally rotated 45 degrees the opposite way (for instance, if the patient’s head was turned 45 degrees to the left, it is now rotated 45 degrees to the right).
  • In the 4th step, the patient should rotate his/her head another 90 degrees in the same direction (in our example, the patient’s head is now rotated 135 degrees to the right) and roll his/her shoulders in that same direction. The goal of the movement is to move the calcium particles from the posterior semicircular canals into the utricle.
