I. Cranial Nerve 12 at Rest
- To assess cranial nerve 12, evaluate the patient’s tongue. Inspect the patient’s tongue, specifically looking for atrophy or fasciculations, and palpate the patient’s tongue (while wearing protective gloves).
II. Cranial Nerve 12 in Action
- Ask the patient to activate his/her tongue as follows:
1. Tongue movement.
A. Ask the patient to stick his/her tongue straight out and hold it.
B. Ask the patient to move the tongue in the vertical and horizontal planes (while continuing to extend it outward).
2. Tongue strength.
A. Ask the patient to push his/her tongue against the side of the cheek.
B. Push against the patient’s cheek and ask the patient to resist.
